Notice of Privacy Practices
This notice describes how your protected health information may be shared and how you can access this information. Please review it carefully.
The Kindness Korner is committed to protecting the privacy of your health information. This Notice of Privacy Practices ("Notice") describes your rights and our legal obligations regarding your protected health information (PHI) under the Health Insurance Portability and Accountability Act (HIPAA) and applicable state laws.
Sharing Your Health Information
Your health information is private and will not be shared without your written consent. A signed Release of Information (ROI) is required before any disclosure of your protected health information (PHI) to another provider, family member, employer, or any third party. Exceptions exist only where required by law — including mandatory reporting of abuse or neglect, response to a valid court order or subpoena, or when necessary to avert a serious and imminent threat to health or safety. You may revoke a Release of Information at any time in writing, except to the extent we have already acted in reliance on it.
Your Rights Regarding Your Health Information
You have the right to: request a copy of your health information; request corrections to your records; request restrictions on how your information is used or disclosed; request confidential communications; receive an accounting of certain disclosures; and receive a paper copy of this Notice upon request. Minors
The Kindness Korner primarily provides individual therapy services to adults (18 and older). However, minors may participate in specific group programs offered through Kommunity Kare, such as Girl Talk. When a minor participates in a Kommunity Kare program, applicable privacy protections and any required parental or guardian consent processes will be communicated at the time of participation.
Breach Notification
We are required by law to notify you if there is a breach of your unsecured PHI.
How to File a Complaint
If you believe your privacy rights have been violated, you may file a complaint with us at hello@thekindnesskorner.com or with the U.S. Department of Health and Human Services Office for Civil Rights at www.hhs.gov/ocr. You will not be retaliated against for filing a complaint.
Changes to This Notice
We reserve the right to change this Notice and to make the revised Notice effective for PHI we already hold as well as any PHI we receive in the future. The current Notice will always be available on this website.
